Admissions & Academic Requirements
Secondary Admissions Policy:
  • Application with parent signature
  • Essay
  • Student Agreement/Scholarship form
  • Completion of necessary credits at Home School
  • GPA of 2.0 or higher
  • Personal Plan of Study
  • Attendance of 90% or better

Adult/Post Secondary Admissions Policy:
  • Application
  • Non-Refundable Application fee of $25.00
  • Copies of transcript (or GED certificate) from all institutions previously attended
  • Interview with Lex La-Ray Technical Center Staff
  • 10th grade or higher reading, writing, and math skills - placement exam may be given*
  • 18 years of age or signed Parental Release Form on file with LLRTC
  • Current on all state required immunizations; records must be submitted with an application

*If the applicant did not graduate from a Missouri Public School, accredited private high school, or receive a GED Certificate, a placement exam will be administered at no cost to the applicant. A passing score is required for admittance.

Licensed Practical Nursing Admissions Requirements:
  • Pass ATI Teas - Pay $70.00 fee prior to taking TEAS if taken at Lex La-Ray.
  • Complete Lex La-Ray Tech Center PN Application with essay
  • $45.00 Application Fee
  • Official copy of High School transcript or  a GED Certificate. A student from a Non-English  speaking country must pass the TOEFL and TSE exams
  • Official transcripts from all post-secondary schools attended
  • Submit two (2) professional reference letters
  • Employee Disqualification List: If the applicant is on the employee disqualification list, he/she is ineligible for completing clinical experiences and therefore unable to complete requirements for this program.
  • Read and sign document MO 335.066

Certified Nursing Assistant
  • Must be at least 18 years of age
  • Completed Application
  • Negative 2 Step TB test within past 6 months - Documentation required at registration
  • $125 deposit paid in full ($100.00 is Non-Refundable)
  • Meet  background check requirements through Missouri Department of Health and Senior Services and the Missouri Highway Patrol

Certified Medication Technician
  • Must be at least 18 years of age
  • Must have 6 months CNA experience or applicable letter of recommendation
  • Meet minimum TABE Test score requirements
  • Negative 2 Step TB test within past 6 months - Documentation required at registration
  • Meet background check requirements through Missouri Department of Health and Senior Services and the Missouri Highway Patrol
  • $175 deposit paid in full ($100 is non-refundable)
  • Copy of Social Security Card - Due at registration

Phlebotomy Technician
  • Complete Application
  • $25 Application Fee
  • Clear background check through Missouri Department of Health and Senior Services and the Missouri Highway Patrol
  • Letter of Intent
  • Letter of Professional Recommendation from an employer, instructor, or co-worker in support of your application
  • Copy of Social Security Card - Required at registration
  • Negative 2 Step TB test within the past 6 month - Documentation due at registration
  • Completed at least first dose, Hepatitis B immunization series (proof of immunization required)
  • Proof of Measles, Mumps, and Rubella (2 immunization dates required)
  • Proof of Varicella immunization (2 immunization dates required or titers with result & date) or medical provider protocol letter
  • Flu vaccination for any individual completing clinicals October - March
  • $200.00 deposit paid in full ($100.00 is Non-Refundable)
